Abstract Submission Temp

Instructions on preparing and submitting an Abstract
Please prepare the following required items:

1. Full contact information for the CORRESPONDING AUTHOR and CO-AUTHORS.
   Author Name*, Affiliation, Specialty*, Phone*, Fax, Email*, Mailing Address*, City*, State/ Province / Region*, Zip / Postal Code*, Country* (Required Fields*)
2. Download and complete one of the following:
   1. STRUCTURED abstract template; or
   2. UNSTRUCTURED abstract template
3. (Optional) Prepare any additional supportive images. Accepted formats are jpg, gif, png, and pdf. Please save to smallest file size of acceptable visual quality.
4. All Abstracts must be in English.
5. Character count should not exceed 2000 characters including spaces. The character count does not include author information.  One figure or chart is acceptable.
