In order to receive your Certificate of Attendance and Credits, please complete the Pre and Post Meeting Survey and Speaker Evaluations for the sessions that you attended.
Certificates of Attendance will be emailed upon receipt of your completed surveys and evaluations starting, 28 January. CME Credits can take up to 4-6 weeks to receive.
If you didn’t complete the attendance sheet and CME form onsite then please download and email or fax those forms to Rose Puleo at rose.puleo@neurotoxins.org or fax # 212-645-1120.
If you do not receive your Certificate of Attendance, please check your Spam folder or contact rose.puleo@neurotoxins.org.
